Our partners operate within the new GMS contract. Our aim is to provide a high standard of quality medical care and we need your co-operation in order to achieve this.

We aim to provide personal family doctoring whilst working together as part of the Primary Health Care Team. Each doctor has a special medical interest. Our doctors are Dr Ochuko Maurice Sanomi, Dr Bunmi Olajide and Dr Oluremi Akin Agbaje.

The College Practice : Rush Green Medical Centre is committed to providing accessible healthcare facilities for our patients.

We are continually making every effort to improve our service and we welcome all your comments and suggestions.

However, if you have a complaint, please write to the Practice Manager in the first instance, who will respond within 48 working hours. A complaints procedure is available on request.

ACR project for patients with diabetes (and/or other conditions)

The data is being processed for the purpose of delivery of a programme sponsored by NHS Digital to monitor urine for indications of chronic kidney disease (CKD).

Annual kidney monitoring may be recommended for patients at risk of chronic kidney disease, including some people living with diabetes. The programme enables eligible patients to test their kidney function from home.

We may share your contact details with Healthy.io so they can contact you and send you a test kit. This can help identify patients who may be at risk of kidney disease and support earlier intervention where appropriate.

Healthy.io will only use your information for the purpose of delivering this service. If you do not wish to receive a home test kit from Healthy.io, your care will continue to be managed by the practice.

Healthy.io is required to retain information in line with applicable records-management requirements for health and social care.

OpenSAFELY

NHS England has established the OpenSAFELY service and the OpenSAFELY Data Analytics Service. These services provide a secure environment that supports approved research, clinical audit, service evaluation and health surveillance.

Each GP practice remains the controller of its own GP patient data. Approved users may run queries on pseudonymised patient information. This means direct identifiers are removed and replaced with a pseudonym.

Only approved users are permitted to run these queries, and they are not able to access information that directly identifies individual patients.

Patients who do not wish for their confidential patient information to be used for purposes beyond their individual care can find information about their data choices through the NHS.

Our practice participates in the NHS Summary Care Record programme. A Summary Care Record contains important information from your GP medical record that can help healthcare professionals provide you with safer care when you need urgent or emergency treatment.

Your Summary Care Record can include information about your current medicines, allergies and previous adverse reactions to medicines.

Authorised healthcare professionals involved in your care may access this information when appropriate, for example in an emergency or when you are receiving care away from your GP practice.

The Freedom of Information Act 2000 (FOIA) gives members of the public the right to request recorded information held by public authorities.
